Student Health Viewing Window

This window shows all Student Health Conditions (Qume Report), for a particular Site, Class and the time span the conditions are valid for

Student Name Full name of student
Site C School Code with which student is enrolled in
Health Condition Short Description of particular Health Condition
Start Date The date the condition is put into the system
End Date The date the condition expires
Comment Any extra statements if needed

Icon Buttons
Searches the existing database
Prints information from the database
Checks all current students for the Health Condition
and updates health verification dates as of today's date..
Allows the user a indepth look at a particular record
Cancel do not write the record/ back to previous screen
