Payroll Profile Query Dialog Window

The Payroll Profile Query Dialog Window allows you to query employee payroll profiles based on a list of criteria base on the fields in the above window.

Input/Display Fields
Staff Type: Pop up menu - For the selection of a specific type of employee;CR = Salary employee with retirement, CO = Salary employee without retirement, ST = Substitute with retirement, DA = Daily rate employee without retirement, SS = Hourly employee with retirement, FI = Hourly employee without retirement.
Date Type: Pop up menu - You can select via Payroll or Payroll Profile dates., In most cases, both dates are the same, but due to payroll situation that the employee may work for 4 weeks then get the first pay, thus payroll profile date and employment profile dates may be different. Thus 2 selections. All following selection concerning dates are affected by this selection.
Start Date Span: Beginning of the Start date's date span
Ending of the Start date's date span
End Date Span: Beginning of the End date's date span
Ending of the End date's date span
Date Active: Pop up menu/Fill in - Allows user to select a payroll profile date: Today, Any, Specific.
Start Reason: Pop up menu allows user select from the reason for starting a new payroll profile.
Termin Reason: Pop up menu - Termination Reason pop up menu for list of all termination codes.
Certificate: Fill-in/Pop up menu - Searchable pop up menu allows user to select a certificate used by the school and an enterable field for the date the certificate becomes active.
Send Check To: Fill-in/Pop up menu - Searchable pop up menu for site to send employee pay check to.
Disability: Pop up menu - for list of disability payroll deduction methods.
Name: Fill-in/Pop-up menu - Searchable pop up menu for selecting employee name.
By Last Name: Check box - Search by last name.
Custom: Pop up menu - this menu may or may not appear base on if custom field is used for specific need
Custom value: This space shall further define the search base on the pop up menu selected.
Fast Title: Radio button - Search by fast title.
SS#: Radio button - Search by employee's social security number.
Variable Text: To be use with the above 2 radio button to be more specific on what is needed to be queried.

Icon Buttons
CANCEL - Cancels the current action and closes the active window.
QUERY - performs a search to select records according to the criteria selected.
