Student Visit Query Dialog

This window is for performing a query on the condition and event records for the students' who have visited a care provider.

Input/Display Fields
Site: Fill-in/pop up menu - Site number.
Provider: Pop up menu - Care Provider's name by employee or non-employee.
By Last Name: Pop up menu - List of providers/employees
Eve. Group: Pop up menu - Event Group list
Cond. & Event: Pop up menu - Condition & Event list
Student: Fill-in/pop up menu - Student Selection pop-up menu w/ selection limiter.
Status: Pop up menu - List by status.
Date: Pop up menu - Date selection pop-up menu with custom selection feature.
Key Code: Radio button - Selection for Key Code.
Care Provider (partial): Radio button - For (partial) care provider name.
Description (partial): Radio button - For description of specific entered notes of the visit.
Variable text: Used by the radio button to perform a search.

Icon Buttons
CANCEL - Cancels changes and closes window.
QUERY - Opens query dialog for selection of care provider.
