Modify Student Health

This is the window for Student Health Info. You can enter the Student's preferred Doctor, Dentist and two Emergency Contacts. It also contains the Health Comments, Conditions, Situations and permission to transport directly to the hospital in case of an emergency.

Input/Display field
Student Name: Display field -The name of the Student
Icon button - Open the contact draggable window.
Select: Pop-up menu Selects the sort order for display of the Contact.
Physician Information
Physician: Fill in/Pop-up menu - The list of contacts If the drag and drop function is used any contact can be made a physician.
Information: Scrollable field - All the necessary information about the contact person.
Button - Gives the user the ability to take a in-depth look that the contact record.
Dentist Information
Dentist: Fill in/Pop-up menu - The list of contacts If the drag and drop function is used any contact can be made a Dentist
Information: Scrollable field - All the necessary information about the contact person.
Button - Gives the user the ability to take a in-depth look that the contact record.
Emergency 1 Information
Emergency 1 : Fill in/Pop-up menu - The list of contacts If the drag and drop function is used any contact can be made a Emergency 1
Information: Scrollable field - All the necessary information about the contact person.

Emergency 2 Information
Emergency 2 : Fill in/Pop-up menu - The list of contacts If the drag and drop function is used any contact can be made a Emergency 2
Information: Scrollable field - All the necessary information about the contact person.
Button - Gives the user the ability to take a in-depth look that the contact record.
Health Comment: Scrollable field - Additional comments regarding the Student's health.
Include Listing Layout of the Health conditions. Group scrollable list - List of all medical conditions associated with the selected student. Along with the health code and start/end date displayed.

Verified as of: Student's health conditions date of verification.
Hospital Permission: Check box -Permission to transport the Student to the hospital in case of an emergency.
