Student Condition & Event Input Window

This window is for entering data about a Student's visit event with a care provider.

Input/Display fields
Full Name: Display field - Student's Full Name.
Condition : Pop-up menu -List of Condition Groups.
Event: Pop-up menu - List of Event Groups
Init. Date: Date of Initial Condition and Event.
Not in Use: Check box - Checked when Student is Out-of-District or Graduated.
Issue Date: Date of Issue (start) of Condition and Event
Expired On: Date of expiration of Condition & Event.
Comment: Scrollable field - Any necessary notes for this student's condition and event record.

Icon Buttons
Cancel - Discards changes to record and closes window.
Go to previous record.
Go to next record.
Write - Writes changes to record and closes window.
