Student Enrollment Input Dialog

This window allows the user to input a new enrollment record for the selected student, modify, or terminate an existing enrollment record. Generally, a student will only posses one enrollment record per site. It is only necessary to create additional enrollment records for students that require a leave from school, then resume attending school. Students that transferred will also require additional enrollment records.

Input/Display field
Full Name: Displays field - Selected students full name.
Site: Displays field - Code of the site/responsibility center where the enrollment record is being created.
Home Dist.: Fill in/Pop-up menu - Students home district site code and site name.
Start Date: This is the date that the student will be actively enrolled for school. By default the system will display the current date, but any valid date may be entered.
End Date: By default, the system will display the students predicted year of graduation. This date is modifiable and is used to terminate a student enrollment record.
Entry Reason: Pop-up menu - This is the reason the student is enrolling at this site. The choices listed in this menu are state specific. Any state that does not require a student entry reason will have no use for this menu, thus the menu will not be selectable. If required by the state, then the selectable pop-up menu will display the state loaded list of entry reasons.
Comment: Scrollable field - Any additional information or comments that need to be attached to the enrollment record.
Leave Reason: Pop-up menu - This is the reason for terminating the student enrollment record. The choices listed in this menu are state specific. The selectable pop-up menu will display the state loaded list of leave reasons.

Icon Buttons
Cancels the action and closes the window.
Records the new student enrollment record or the modifications made to existing records to the database.
