Add Health Condition

This window adds the students health conditions to the student data for references

Input/Display fields
Full name: Display field - Students full name (Last,first and middle name)
Health condition: Pop-up menu - Choice of health condition from a pop up menu.
Start Date: Start date when health condition is implemented
End Date: End date of the implementation of the health condition
Comment: Scrollable field -Additional comment on the student health condition

Cancel/Escape This allows the exit of this window and cancel all input if not saved
Write This saves the current inputs in this window.
