IEP Team Input Window

The IEP Team Input Windo which allows the user to input the IEP team member to an IEP student. This window includes the following data fields and command buttons:

Input/Display Field
Heading The students ID number and the name of an IEP student.
Member Type Pop-up menu - The type of the IEP member, includes employees, non-employees, specialists, and contacts.
Name Fill-In/Pop-up menu - The name of the IEP member.
By last name Check box - Search for the IEP member by the last name.
Start date The date of a person became an IEP member for a perticular student.
End Date The date of a person not an IEP member for a perticular student.
Comment Scrollable field - box of comment about the IEP member.

Icon Buttons
Cancel/Escape Button cancels the IEP team input operation
Write Button accept records input to the database.
