Contact Setup Dialog

This is the Sort Window which allow the user to choose the format of the data to be displayed

Buttons
Split Address - the system will automatic split the address of a contact to be display. Split with the number, direction, street name, and street description.
Emp. Update - update the contact files based on the current employee file
Flag - the system flags the doctor, dentist, and the related for current use(update the system as a contact is a doctor, dentist, and related).
Cancel - cancel the contact setup operation.
